(ON Only) The Prescriber Tab

The Prescriber tab has been updated to document information that will be provided to the patient’s healthcare team. In addition to the Primary Prescriber, Secondary Prescribers that you would like to communicate with can be added to this tab. In this tab you can:

• Change the Primary Prescriber
• Add or Remove a Secondary Prescriber
• Fax or Print the Prescriber Notification Form
• Fax or Print the Diabetes Education Checklist Form
